The Forms below are for your reference. All of these will either be emailed when you schedule your first session, or a paper form to sign during your first session. You are welcome to print and sign and bring with you, but no worries if not.

Fusion’s Waiver of Liability must be read and signed before participating in services. This form helps ensure you understand the potential risks, responsibilities, and expectations associated with your visit and participation in services at Fusion.
Fusion’s SMS Communication Policy informs about how we use text messaging for client communication. This form outlines what you can expect from Fusion’s text messages, including scheduling, reminders, updates, and communication guidelines.
We accept the following payment types:
Cash, Check, Credit Card, Debit Card, Venmo, Google Wallet, Apple Pay

A 3% surcharge is added to all Credit and Venmo payments
No surcharge for Debit, Cash, Check
Returned Check Fee: $25
