By booking or participating in services at Fusion Bodywork & Fitness, LLC, you agree to the following terms and conditions:

Appointments & Booking

Appointments are subject to availability and are not considered confirmed until they have been booked through our scheduling system or confirmed directly by Fusion. Please provide accurate contact and health information when booking and completing required intake forms.

Cancellations & Rescheduling

Please review Fusion’s current cancellation and rescheduling policy before booking. Late cancellations, missed appointments, and no-shows may be subject to applicable fees or loss of the session. Repeated cancellations or missed appointments may affect your ability to book future services.

Health & Safety

You are responsible for providing accurate and complete information regarding your health history, medications, injuries, medical conditions, pregnancy status, and any other information that may affect your ability to safely participate in a service.

Fusion may modify, postpone, or decline a service when necessary for your safety or when a service is not appropriate based on the information provided.

Certain services may require healthcare provider approval or additional documentation before participation.

Services & Results

Services provided by Fusion are intended to support wellness, fitness, movement, relaxation, and general well-being. Services are not intended to diagnose, treat, cure, or prevent medical conditions unless specifically provided within the scope of the provider’s professional license.

Individual results vary. No specific outcome or result is guaranteed.

Client Responsibility

Clients are expected to communicate openly with their provider about comfort, pain, health changes, and any concerns before or during a session. Please immediately notify your provider if something feels painful, unsafe, or otherwise inappropriate during your service.

Clients are also responsible for following instructions provided before, during, and after their service.

Age Requirements

Minors may only receive services when permitted by Fusion policy and with any required parent or guardian consent. Certain services may have additional age restrictions.

Payments & Packages

Payment is due according to the terms of the service booked. Prepaid packages, memberships, gift cards, and promotional offers may have specific expiration dates, restrictions, eligibility requirements, or other terms. Please review the applicable terms before purchase.

Unless otherwise stated, prepaid services and packages are non-refundable and may not be transferred.

Personal Belongings

Clients are responsible for their personal belongings while at Fusion. Please avoid bringing unnecessary valuables to your appointment.

Right to Refuse Service

Fusion Bodywork & Fitness, LLC reserves the right to refuse, discontinue, or modify services when necessary for safety, professional boundaries, inappropriate behavior, failure to follow policies, or other circumstances that make providing the service inappropriate.

Changes to Terms

Fusion Bodywork & Fitness, LLC reserves the right to update these terms and conditions, policies, services, pricing, and scheduling requirements as needed. Updated terms will apply to future bookings and services.

By booking an appointment or participating in a Fusion service, you acknowledge that you have read, understood, and agreed to these Terms & Conditions and any additional policies or forms applicable to the service you are receiving.
